Nurse Care Manager (Clinical)
Collaborate with primary care providers, clinic nurses, public health nursing, DM case management and social services to ensure that the patient's care needs are completed in a timely and cost effective manner to maximize each visit. Serve as primary contact/clinical liaison between referral hospitals, treatment facilities, and patients and their families to ensure delivery of effective and appropriate treatment in accordance with established plan of care. Contact patients for follow-up diagnostic tests, referrals, appointments, or other medical issues as appropriate in collaboration with the provider. Interact with the patient/family/caregiver and primary care provider to develop and discuss the patient's plan of care, understanding of disease processes, and coordination of services based on patient's clinical needs and available resources. Participate in departmental orientation, on the job training, and quality improvement programs/initiatives. Coordinate clinic visits for assigned patients with other members of the multidisciplinary team in order to optimize care. Conduct intake assessment, screening, and follow-up interviews with client and/or family members and/or partner(s) at office, by telephone, in the clinic, or at the home of the client. Basic Requirements Education A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education , Council on Accreditation of Nurse Anesthesia Educational Programs , Accreditation Commission for Midwifery Education , or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School:
Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Advanced Practice Nurse Positions:
For Advanced Practice Nurse (APN) positions, applicants must possess a graduate or higher level degree in nursing from an accredited educational program in the area of expertise or specialty indicated by the position. is defined as experience in performing comprehensive clinical assessment skills in the adult population and chronic disease management in addition to serving as a consistent member of a multidisciplinary team whose purpose is to conduct care coordination for patients beyond what is offered in the immediate facility (i.e. home health care, hospice care, rehabilitation therapy, skilled nursing facilities, specialty services, durable medical equipment (DMEs), tribal resources, etc.). You must meet all qualification requirements within 30 days of the closing date of the announcement.
